Summary03606: all questions become "hidden" when radio button or checkbox answered on "Test this survey"
Description

Survey has 16 questions, types(0:15)=(Z,N,M,Z,T,Z,Z,Y,Z,Y,Z,Y,Z,Y,Z,Y). Of q(0:15) q4, q6-q15 all have conditions. Initially, q0-q3 & q5 are displayed; q4 & q6-q15 are hidden. When I answer q0, q2 or q3 -- ALL questions become hidden. However, q5 can be answered successfully, then q0, q2 or q3 can be answered.

this only occurs when lime_surveys.format = 'A' (All in one). Everything works fine when format = 'G' (group by group).

Hi, your LimeSurvey build number is VERY LOW ==> please upgrade to the latest stable plus version and tell us, if you can reproduce the issue.

At my installation (=latest Build) everything looks good to me:

upgraded to build 7548 which works correctly on localhost, I will move to server and advise outcome.
