Description

When using "List with comment" question type with "Dropdown presentation" option set to On, it doesn't offer placeholder value, meaning the question can never be unanswered.

Also there is a small layout issue. Please align the text box with the dropdown (the issue is shown in the screenshot).

  1. Create a survey with "List with comment" question type and add some options to it
  2. Set "Use dropdown presentation" from Display options of this question to ON
  3. Go to Settings -> Presentation and set "no answer" to OFF
  4. Save and preview the question
  5. The question's first option is already preselected

Expected result

There is "Please choose..." shown in the dropdown like in the "List dropdown" question type

Actual result

The first option is already preselected and I can't leave this question unanswered.
