Depending on the cursor position on the left navigation, when you add a new question it might give you a page that has no option to save the question you are adding. It is reproducable, but hard to be specific as to where the focus is when this occurs. This occurs when clicking the "add question" button. Try setting the cursor focus in the left navigation, but not selecting a specific group. See the attached screen shot.
