Summary14285: the question type to upload a file does not have a button to select and upload files....

when executing a survey with a question type to upload a file than there does not appear a button to select and upload files ....

I'm unable to reproduce this issue.
If you are running LimeSurvey on your own server, this issue may be related to server configuration or permissions.
You should set 'debug'=>2 in your config.php file.
Also turn Developer console in your browser (F12).
If there are any error messages, please post it here.

I turned the Developer console on and got following error (also see screenshot) in my browser.
Load denied by X-Frame-Options
I solved the problem by editing /etc/apache2/conf-enabled/ssl-params.conf and replaced the 'Header always set X-Frame-Options DENY' with 'Header always append X-Frame-Options SAMEORIGIN' and restarted the apache service.
I'm using Apache/2.4.29 with PHP Version 7.2.10-0ubuntu0.18.04.1
