Summary14120: Can't upload image when building survey
Description

Actually have Version 3.14.11+180926 installed.

I recently installed a fresh version of Lime Survey. I was using 2.06 without problems, but now version 3.14.11 has a problem.

When I try to upload a photo to display in my survey, I am unable to browse the server to upload the photo. I get a blank window instead.

Steps To Reproduce

Click the image icon (in editor menu)
click Browse Server next to URL
Popup box shows "/third_party/kcfinder/browse.php?type=images&CKEditor=description_en&CKEditorFuncNum=0&langCode=en" but the window is blank

Additional Information

I do not proclaim to be an expert at this, but have used version 2.06 for years without problems on the same server...

I'm able to reproduce this issue only when survey upload folder can't be written by web server.
Survey images are saved to folder /upload/surveys/SURVEY_ID/images/.
So, solution for this issue is to make folder /upload/surveys/ and subfolders writable with recursive option.

I also had the same error on the version 3.15.1+181017.
Now on the version 3.15.5+181115 it seems that the problem is solved! After the update I can upload the images without errors.
