I did another test run with the latest shipped "default" template using our template test survey. There are several issues:

ALREADY FIXED 1. "No answer" being placed outside question column, see attached screenshot 1.png.

ALREADY FIXED 2. Radio buttons and answer options (column headings) at array questions being left aligned and not centered within column, see attached screenshot 2.png.

ALREADY FIXED 3. Missing radio buttons for "10 point array" question type, see attached screenshot 3.png.

FIXED 4. Dual scale headers should be centered above column headings. Maybe we should also think about adding horizontal lines to make it easier for the user to differentiate the different scales. Currently that is hardly visible, see attached screenshot 4.png.

FIXED 5. Date picker button at date question should be placed right to the input box.

CANNOT REPRODUCE 5-2 Also when entering a valid data an empty warning pops up, I have no idea why, see attached screenshot 5.png.

6. Maybe we should add some spacing between the ranking containers? Maybe we could also take care that the containers keep their height? Currently, they automatically adjust so when having ranked all items the left container is suddenly using less height, see attached screenshot 6.png.

FIXED 7. Delete button at file upload needs a little padding, see attached screenshot 7.png.

8. Different item colors at gender and yes/no question elements are hardly visible. Maybe we should also add some padding between the elements, currently it looks like a whole bar, see attached screenshot 8.png.

CANNOT REPRODUCE 9. The layout of the multiple numerical input question is a little messed up, see attached screenshot 9.png.

10. Some more issues with the multiple numeric input question:
    NB: That's because the bold error messages is the active one. Will try to improve. a) the error messages use different styles, that looks a little weird.
    FIXED b) prefix and suffix are completely missing
    FIXED c) there is a list item at the bottom
    ..., see attached screenshot 10.png.

FIXED 11. When setting a "equals sum value" at the advanced question settings, the white color used for this is hard to read and font size is too small and there is that list item shown as well, see attached screenshot 11.png.

12. Sliders are missing a "fill color"
    FIXED: and have the same issues as described at #11 when setting a total sum, see attached screenshot 12.png.

13. Lengthy answer texts at multiple choice questions partly float below the checkbox which makes it hard to read them, see attached screenshot 13.png.

14. At multiple options with column questions lengthy answer texts completely float below the checkbox, see attached screenshot 13.png.

15. At multiple short questions the text boxes should be next to the item texts, currently there is lots of space between them.
    Also the prefix and suffix should be placed before/after the text field, see attached screenshot 14.png.
