When survey takers upload files, it looks to the survey taker like they are saved once they click "Return to Survey". Some users completing persistent token surveys have been closing their browser after several file uploads, to come back and complete later, without scrolling down to click on "Resume later", "Next", "Previous", or "Submit" as intended.

The result is files successfully uploaded to the server that aren't associated with a database record.
