I have this issue too, but no time to report. More quick to fix theme file before upload

The theme file is correct before upload. After uploading, it gets modified.
The best solution is to place the file directly on the server instead of using the application to install it.

So I can confirm that with Denis' really_hidden_equation question theme, it puts another, double version 5 in the config.xml file.

And for https://bugs.limesurvey.org/view.php?id=20372#c85141 Net-Promoter-Score-2.zip (must rename zip file) add an uneeded 5.0
