Summary16581: Problem with file upload functionality
Description

If the form has multiple file upload fields, the functionality stops working, both the "delete" button and the "select file" button stop working.

The first upload files button works (File upload 1 in picture below), but if you try to upload another file, for example add a new File upload 2 or File upload 3 the button select file stops to working in this fields, see pictures and file below.
