Summary07867: 5 point scale question type is missing CSS classes for slider or star rating layout
Description

When using the 5 point scale question type and setting the layout to use the star rating or slider, the generated question HTML is missing CSS classes for the additional GUI elements of that design.
This is a pity, because you are not able to adjust the layout using CSS and you can't really use these designs when using a custom template.

Steps To Reproduce

Create a survey with 5 point scale question and use the slider or star rating layout at the advanced question settings.
Then check the questions source code.

Fixed in 2.0.

2.05 not affected - it looks like someone is reworking the 5-point slider and stars in 2.05 there.
