Summary06494: Attribute field isn't created
Description

When importing a survey which has in token table additional atributes they aren't created, but when a user try to add new participant in tokens table field is shown.
For my option also deleting of attributes should also be added.

Steps To Reproduce
  1. import a survey
  2. create token table
  3. check additional atributes (it show no records)
  4. try to add user to token table (last line is missing additional atribute)
  5. when clicking Add, error is shown: Property "Tokens_dynamic.attribute_1" is not defined.
