I updated the LS and ran the script again but ended up with a bunch of IDENTITY_SET OFF error. It is trying to insert some records into 'lime_questions' table with the 'id'.

So, I set the IDENTITY_INSERT to ON, restore the DB, ran the scripts, and I still got the same errors.

So I ignored it and logged into it, deactivate the survey, change settings, and reactivated it. This time, it activated without any errors. But when I clicked "Next" after the first question, I got the following error.
