Survey theme/template: fruity

1.Add a new attribute in CPDB.
2.The default attribute name is set as "工號"(Non-ASCII characters).
3.Add the English language "Employee ID".
4.Switch the user interface language to English.
5.The attribute in CPDB remains as "工號" instead of "Employee ID".
