Summary	16806: When in All in one mode id's are rendered duplicate
Description	When a survey is taken in 'All in one' mode the produced HTML contains duplicate java{sqid} values.
Steps To Reproduce	Take a survey in 'All in one' mode and view in browser with F12 (all modern browser show warnings 'Found 2 elements with non-unique id ...) or view source.

"'/>\n"; or maybe the produced hidden input is obsolete?
