Can you please clear the Yii cache and try again? Remove all content in folder <webroot>/tmp/rutime/cache/

I stopped Apache, removed everything from the cache-directory you said, started Apache and opened a private browsing window. No luck, still the same error. And this is seen in a survey having just one question and one response top that with one file attachment.

No reason to stop Apache or start a private browsing window. Clearing the cache folder is enough. Thanks for testing.
