| Summary | 13726: Error Msg -'Failed to Add New User' |

| Description | Hi LS.
I am attempting to add/create a new survey user in newly installed LS3.8.1
When I add a new survey user, LS generates a ‘failed to add user’ error msg, LS does not generate the password email.
–But when I check under ‘Manage Survey Administrators’, I see that the new user is displayed as created. However I/user did not get the email from LS with the password.
No reset password email either, via the 'Forgot your password?' option.
Pls check.

| Steps To Reproduce | 1)Log in to In LS 3.8.1
2)Click on 'Configuration'
3)Under 'Users' Choose/click on ‘Manage survey administrators’
4)Then Click on the '+' sign that says 'Add user'.
5)When the new pop up window 'Add a new survey administrator' shows up, Fill in the 3 required info & click 'Save'.
This is when I get the error msg. |

| Additional Information | I do see the 2 new users are created/recorded in [LIMESURVEY381].[dbo].[lime_users] table.
I tried to reset/get the new user password using the 'Forgot your password?' option, But LS did not generate the temp password email either. |
