Summary12812: Attaching a file to an email template does not work
Description

When trying to attach a file to an email template, the operation fails in IE10, FF 55 and Chrome 61.
I get no error message, nothing happes on button click

Steps To Reproduce
  1. In any of your surveys, open your email templates
  2. Choose any template
  3. Click on "attach file"
