The problem is your template. You are probably missing a CSS class "hidden" in it, am I right?

Try to add this to your CSS:

.hidden {
display: none !important;
}

I don't know why it's working but it's a fact.
I've put your lines in the section introduced by
/ DEFINED ABOVE UNDER "General list styles" - - - - -
.multiple-opt
/
in template.css ... and the bug has disappeared

Mystery remains that the bug does'nt exist under limeservice with the same template, but I don't need to know everything !
I'm so glad that my questionnaire is now operationnal !!!
