removed old 'answer' editing and adding stuff, changed to new. Fixed sortorder so sortorder field cannot be directly edited. Removed messages for moving up and down. Reformatted table for up/down in "move" column and save/del in "action" column. Adjusted so codes and answers cannot be modified / deleted for all question types other than "List" when survey is activated.
