Fixed some MSSQL insert problems

MSSQL don't allow by default insert values in an identity field (autoincrement in MySQL) that happend when adding groups and adding questions

MSSQL don't accept the sentence: INSERT INTO tablename SET field=value instead of that we should use always INSERT INTO tablename [(list of fields)] VALUES (values)
