Form fields are missing own label, failing criterion 2.4.6. Page is not accessibile.
Every field should have a label.

Steps to reproduce

  1. instal latest LS
  2. create a few surveys
  3. inspect code in the page admin/dashboard/view
  4. see form fields are missing own label.

Expected result

Every form field should have its visible label

Actual result

Fields don't have a label. Checkboxes inside the table, and select fields also, for example.
If fields can't have a label, should be removed. checkboxes, for example.
