Summary05528: Timer attribute doesn't work at survey taking. Notices at question preview
Description

When testing a survey with a timer attribute (http://docs.limesurvey.org/Advanced+question+settings&structure=English+Instructions+for+LimeSurvey#time_limit) there is just a black bar instead of a box showing the timer at survey preview.

When previewing the question only, I receive the following notices:

Steps To Reproduce
  1. Import the attached survey.
  2. Test the (deactivated) survey. On question page one (group "Text question") check question "Short free text (Numbers only, With a timer)". You only see a small black bar there (see attached screen shot).
  3. Preview the according question only to see the notices mentioned above.

Removed the warnings in r11247. This may hide the cause.

dionet, I believe this is related to bug 5470

quanda_helper - line 5137 is not working:

$js_header_includes[] = '/scripts/coookies.js';

I suspect that there are several other instances of $js_header_includes[] also not working.
