Description

Even though I have defined Group titles and descriptions, they are not showing up in my survey when I run them.

Steps To Reproduce

The template I am using is a modified version of "basic," but I have tried applying other templates (e.g. Limespired) and they also do not display the Group information.

I have checked, and it is also happening with both inactive and active surveys.

FYI, I noticed that the groupname is omitted from the title bar of the window as well. So perhaps it's not a template problem, but rather a problem in the handling of the Group Name field?

I included both a sample survey and some screen shots of what it looks like with the "basic" template. The error also occurred with the "default" template.
