Summary19450: Unable to create a new survey.
Description

We recently switched from using MySQL to using MS SQL Server. Now we can not create a new survey.

Steps To Reproduce

Click on 'Create Survey'

Enter 'Survey title'.
Click on 'Create Survey' or 'Save'
The 'Create, import, or copy survey' screen comes back with the 'Survey title' blanked out. Nothing is added to the database.

Expected result

New Survey Created

Actual result

No survey created.
