Steps to reproduce

1. Browse to survey menu question list of
2. Select one or more questions from the list using checkboxes on the left
3. Select option "Set question group and position" from the mass actions select box on the bottom left of the list
4. Position select box may or may not appear
5. Close modal
6. Select one or questions again
7. Select option "Set question group and position" from the mass actions select box on the bottom left of the list
8. Position select box may or may not appear (intermittent display)

Position select box should always be displayed in the modal
