Seems that moment.js is not included.

Found only moment-with-locales.js in yiiweels

bootstrap-datetimepicker.js:49 Uncaught bootstrap-datetimepicker requires Moment.js to be loaded first

The priority and implication is severe!
In some of our surveys the date is mandatory.
The date field doesn't work, even manualy no date entry is possible.
Therefore, the surveys are broken/impossible to finish.

Workaround:
Changed to default template

and copied
https://raw.githubusercontent.com/smalot/bootstrap-datetimepicker/master/js/bootstrap-datetimepicker.js

to templates/default/scripts

deleted my session in tmp/assets

I can reproduce this issue with my template, see http://ls25.mysurveyhosting.com/index.php/survey/index/sid/459676/newtest/Y/lang/en

404 error for loading http://ls25.mysurveyhosting.com/tmp/assets/bf7cb909/js/bootstrap-datetimepicker.js

The temporary solution to remove the assets folder worked for me.
