How to provide accurate feedback:


NEW THIS YEAR: Questions and rating scale to help you provide even more accurate feedback.

 

  • You will rate this person on a 7-point scale, ranging from "Never"  to "Every Time", based on how often you see them demonstrate different work behaviors.

 

  • Answer each item based on specific behaviors you have observed from this person on a regular basis over the last 12 months.  Do not rate based on what you hear others say.

 

  • Rate the person on each question independently instead of being influenced by ratings on other items.  Don’t be afraid to use the entire scale where appropriate.


  • If you feel you don't have enough information to provide a rating on a certain work behavior, choose the "NA/Unable to Observe" option.


  • In written comments give specific behavioral examples and suggestions to help this person grow to their full capabiliy

How to take the survey:


  • To begin the survey, click "Next" at the bottom of the page.
