Summary06146: "Load a previously saved survey" screen problem
Description

The "Load a previously saved survey" form is not wrapped in an admin page. See attached screenshot.

Steps To Reproduce

Load, activate and run attached survey. Click "Load unfinished survey".

There don't seem to be any HTML or JS errors.
