Summary05678: useslet field is not created by Remove Control in Tokens table
Description

When participants are created using the remote control client, method createTokenTable in lrsc.helper.php doesn't create the usesleft field which causes the tokens to be considered invalid.

Steps To Reproduce

1) Using the test client, create survey 29846
2) Using the test client, insert some participants
3) Using the admin interface or the test client, activate the survey
4) Look at the tokens created in the admin interface and try to complete a survey with it. You'll get a message telling your token is invalid.

Additional Information

I know this isn't keep up to date, but as we needed this feature I've fixed this issue in my project. The correct file is attached to this bug report.
