Summary04879: Missing image files
Description

After a clean install of LS 1.91RC2, when I login for the first time and select the 'Default Administration Page' to create the first survey, the broken image placeholder appears next to Step 4, 'Done. Test your survey...'. If I right-click the placeholder it is looking for an image named do_20.png. If I look in the images directory that file does NOT exist. Looks like it may be missing from the distro file.
