04564: Question appears in print view but doesn't appear in on-line view

Description

As far as I'm aware I entered this question (ranking) in exactly the same way as all the others, and it appears fine in the printout view from the admin screen, seems fine when I edit the question, but never appears in the on-line survey or the results.

Have set as urgent as this is a complete disaster for me to find at this stage, and I'm sure it would be for anyone else. I have exported an lss and imported and the question behaves identically, ie, doesn't appear, so I hope you can reproduce it. I'd really really appreciate it if you could let me know how to fix it, (ie, line number and code change, or email me the new files, or whatever's easiest). I assume the answer columns aren't being created, but I could always take off-line, do the fix and re-activate.

Steps To Reproduce

Import the attached lss file.
Click the printable version.
Look at section 2 question 5 forms of power on the printable version.
Run the survey (activated or not), go through to section 2 and notice that the question isn't there.

(12716) technicaltitch (reporter) 2010-08-26 17:49	As far as I can remember, I created the questions, then put them into sections, then re-ordered them, in case that helps identify where the error happened. I don't remember anything special about this question, and I've plenty other ranking questions working fine.
