Summary04394: Field to enter assessment code is missing
Description

When having assessments enabled at survey settings and trying to add assessment values for a list (radio) question the field to enter the assessment value is missing.

Steps To Reproduce
  1. Create simple survey and enable assessment mode at survey settings
  2. Add Group and a list (radio) question
  3. When adding answers there is no field to enter assessment values
